Who we are:
- A collaborative community based group that helps families get connected to services and needed resources such as; financial, emotional, psychological, housing, medical, recreational, educational plus more...
What we can do:
- Help develop an action plan for each child and family based on their individual needs
- Facilitate communication and collaboration among child and family service agencies
When is it needed?
- When additional support services are needed to help the child be successful in the community
- When parents and/or others have concerns about the child's ability to remain at home
- When a child is returning home from a residential placement or long-term hospitalization
